“Bundle of Smiles” Extended Care Addendum

Based upon results of initial standard hygiene visit, if patient is diagnosed with periodontal disease, additional treatment of 4 quadrants of deep cleaning (scaling and root planing) would be required and patient would not be eligible for ZOOM Whitening services. After deep cleaning service, patient would make 3-month hygiene visits for a minimum of one year upon which their oral health will be re-evaluated.

An additional program cost of $759 would be required for periodontal disease treatment. This cost would be in addition to your basic dental care package.

Disclaimer: To be eligible for the Bundle of Smiles small business dental care plan, you must be a small business owner. One year service agreement required to participate in plan. Plan costs can be paid in full or made in 12 monthly recurring payments. If patient is found to have periodontal disease, patient will be transferred to Extended Care Plan for an additional cost.
